Do I need to be referred by another physician?
No. 420 MediCard can evaluate any person who feels that they can benefit from medical marijuana.
Will I be listed on a federal or state list if I get a marijuana recommendation?
No. Your medical records are covered by Federal HIPPA regulations.
What is the process of obtaining a recommendation?
As a paperless facility you will are encouraged to complete the online form prior to arriving to our facility. However if you are unable to do so you can complete the form at 420 MediCard. In addition to completing the form you will be asked to provide a valid state-issued ID or driver's license. You will then be evaluated by one of our compassionate board certified physicians. If you qualify you will receive your recommendation during that visit.
Do I need to bring my medical records or previous recommendation?
Yes. It is important to bring in any medical documentation that you may have such as medical records, ER visit, chiropractor visit, X-Ray, MRI, prescription or previous recommendation.
How do I get my medical records?
Contact your physician or healthcare facility where you were treated and request that your records be released to you. Typically they will ask you to sign an authorization after which your records will be faxed, mailed or released to you.
How old do I have to be to receive a recommendation?
You need to be 18.
Where do I get medical marijuana if I receive a recommendation?
420 MediCard cannot assist you in obtaining marijuana nor can we direct you to any dispensary, collective or cooperative. There are numerous websites, organizations and publications that can help you.
How long does my recommendation last?
Your recommendation will last for 1 year from the date it was issued.
What happens if I lost or cannot obtain my records or documentation?
You can still be evaluated at 420 MediCard. Our physician may offer you a temporary recommendation for a period of 1 to 3 months depending on what information is needed. We have a referral list of free clinics if necessary. |
